For inclusion in the Rescue Listings on the Canada's Guide to Dogs Website

Please complete the following questionnaire to be included in the Rescue Organization Listings on the Canada's Guide to Dogs website. Once completed, click on the submit button to process the form.

Note: The questionnaire is for internal use only and the information received will never be passed on to any individual or organization. It is our goal to provide the opportunity for legitimate, not-for-profit rescue organizations to be included in the listings. However, due to a concern of unintentionally including organizations who are questionable in their rescue practices, we request that Rescue Organizations complete the following questionnaire prior to being included on the website. We do not recommend, endorse or support any one organization over another and we will never knowingly list an organization who runs a puppy mill, backyard style breeding operation, or sells their dogs (or any animal) to brokers, pet stores or retailers of any kind. We reserve the right to refuse any listing/link as well as the right to immediately remove the listing/link of any organization who has already been listed at our sole discretion.

The following information is for our own purposes only and will be kept strictly confidential.

  1. Are the rescued dogs examined by a veterinarian, brought up-to-date on vaccinations, and assessed for temperament? If not, why?
  2. Are the rescued dogs spayed/neutered before being placed in their adoptive homes? If not, why? and if not, does the adoption contract specify that, unless for medical reasons, the dog must be spayed/neutered?
  3. Do you have a detailed adoption application whereby prospective new owners are verified through references and other means to ensure that the dog will be going to a permanent and loving home?
  4. Are you willing to take back any animal that is placed if the placement does not work out for whatever reason?
  5. Do you, will you, or have you ever used any of the rescued dogs for breeding purposes?
  6. Do you, will you, or have you ever sold rescued dogs to brokers and/or retailers?
  7. Have you ever been subjected to disciplinary action, fines, etc., by any dog club, criminal or civil court?
